01 | Privacy, Data & Technology

Data Collection & Privacy Policy

Effective Date: August 1, 2026

OPPMed respects the privacy of the individuals, patients, employers, organizations, and other persons who interact with our practice. We collect and maintain information only as reasonably necessary to provide healthcare and occupational medicine services, administer our operations, communicate with users, process payments, comply with legal and regulatory obligations, maintain security, and improve our services.

Information collected may include identifying and contact information, demographic information, health and medical information, medications, health history, laboratory and imaging information, appointment information, communications with OPPMed, payment and transaction information, employer-related information when applicable, and technical information generated through use of our digital platforms.

OPPMed does not sell personal information or protected health information. We do not disclose patient information to third parties for their independent advertising or unrelated commercial purposes. Information may be disclosed when reasonably necessary for treatment, healthcare operations, payment processing, legal or regulatory compliance, or delivery of requested services. This may include appropriately authorized laboratories, pharmacies, imaging facilities, healthcare professionals, technology providers, payment processors, professional advisers, and other service providers supporting OPPMed operations.

Where HIPAA applies, uses and disclosures of protected health information are governed by OPPMed's separate HIPAA Notice of Privacy Practices. HIPAA generally permits covered providers to use and disclose information for specified purposes such as treatment, payment, and healthcare operations, while giving patients important rights concerning their information.

OPPMed maintains administrative, technical, and organizational safeguards designed to protect information against unauthorized access, alteration, loss, misuse, or disclosure. Patients are responsible for providing accurate information and for maintaining the confidentiality of their account credentials and devices. Website Privacy Policy

This Website Privacy Policy describes information collected through OPPMed's website and related online services. When you voluntarily submit information through our website, we may collect your name, email address, telephone number, address, appointment information, inquiry details, registration information, and other information you elect to provide.

Our website and technology providers may also automatically collect limited technical information such as browser type, device type, operating system, IP address, referring pages, pages visited, session information, and similar technical data used to operate, secure, troubleshoot, and improve the website.

Information submitted through a clinical intake form, patient portal, telehealth platform, or other designated healthcare system may become part of your medical or administrative record and may be subject to additional privacy protections. OPPMed does not sell website visitor information or patient information.

Our website may contain links to websites operated by independent third parties. OPPMed does not control the privacy practices, security, accuracy, availability, or content of third-party websites. Users should review the privacy policies of those services before providing information. Use of this website constitutes acknowledgment of this Website Privacy Policy.

Cookie & Tracking Technology Notice

OPPMed may use cookies and similar technologies necessary to operate, secure, analyze, and improve our website. These technologies may support functions such as remembering user preferences, maintaining secure sessions, identifying technical problems, understanding general website usage, preventing fraud, and improving website performance.

OPPMed does not knowingly permit tracking technologies to use protected health information for third-party targeted advertising. Users may be able to restrict or disable cookies through their browser or device settings. Disabling certain cookies may affect website functionality.

Because healthcare websites can involve particularly sensitive information, OPPMed evaluates the technologies used on its website with consideration for applicable healthcare privacy requirements.

Electronic Communications Notice

By providing an email address, telephone number, or other electronic contact information to OPPMed, you authorize OPPMed to communicate with you electronically regarding appointments, registration, billing, administrative matters, requested services, and other practice-related communications, subject to applicable law and your communication preferences. Electronic communications may include email, text messages, telephone calls, patient-portal messages, or other approved communication methods.

Electronic communication carries inherent risks. Messages may be viewed by individuals with access to your device, telephone number, email account, or notification screen. Patients should not use ordinary email or text messaging for urgent or emergency medical concerns. OPPMed may require sensitive clinical information to be exchanged through a secure platform rather than ordinary email or SMS. You are responsible for keeping your contact information current and for protecting access to your devices and accounts.

Telehealth Technology & Security Notice

OPPMed provides healthcare services through remote communication technologies. Although OPPMed uses reasonable safeguards designed to protect patient information, telehealth carries inherent technological and privacy risks, including internet interruptions, unauthorized access to improperly secured devices, inadvertent disclosure to individuals near the patient, and technical failures.

HHS specifically advises telehealth patients to use private locations, secure personal devices, strong passwords, updated software and secure networks, and to avoid public Wi-Fi when communicating sensitive health information.

OPPMed may verify a patient's identity and physical location before or during a telehealth encounter. HHS telehealth guidance identifies identity and location verification as appropriate safeguards in virtual care. Patients should participate from a reasonably private location whenever possible and should notify their clinician if another person is present.

OPPMed does not routinely record telehealth encounters. Any recording, when necessary and legally permissible, requires appropriate authorization and safeguards.

Data Security & Breach Notification Notice

OPPMed maintains administrative, technical, and physical safeguards reasonably designed to protect information entrusted to the practice. No information system can be guaranteed to be completely secure. If OPPMed discovers a breach or other security incident involving protected or legally regulated information, OPPMed will investigate and provide notifications as required by applicable law.

HIPAA-covered entities are subject to federal privacy, security, and breach-notification requirements, while certain health technologies outside HIPAA may instead be subject to the FTC Health Breach Notification Rule. Individuals who believe their OPPMed information or account has been compromised should contact OPPMed promptly.

02 | Appointments, Payments & Cancellations

Appointment Cancellation Policy

OPPMed reserves clinical time specifically for each scheduled patient. Patients who need to cancel or reschedule an appointment should do so as soon as reasonably possible through the scheduling system or by contacting OPPMed. Cancellation of an appointment does not automatically create a right to a refund.

OPPMed may cancel, postpone, or reschedule an appointment when necessary because of clinician availability, emergencies, licensure restrictions, patient-location restrictions, technology failure, incomplete registration, inability to verify identity, or other circumstances preventing appropriate care. If OPPMed is unable to provide a paid standalone service and no reasonable alternative appointment can be offered, OPPMed may issue a refund or account credit as appropriate. Failure to attend an appointment does not constitute cancellation.

Standalone Appointment Refund Policy

All standalone appointment fees are non-refundable once payment has been submitted, except where otherwise required by law or where OPPMed determines that it cannot provide the purchased service. This includes missed appointments, patient-requested cancellations, and situations in which the clinician determines after evaluation that the requested treatment, prescription, laboratory order, imaging order, certification, referral, or other intervention is not medically appropriate.

Payment purchases professional evaluation and clinical decision-making. Payment does not guarantee a particular diagnosis, treatment, prescription, order, certification, referral, or medical outcome. A patient's disagreement with a clinician's medical determination does not create a right to a refund.

Direct Primary Care Subscription Refund Policy

OPPMed Direct Primary Care memberships are offered on monthly and annual subscription terms. The $250 enrollment fee is due at enrollment and is non-refundable. Monthly membership payments are non-refundable once charged. Monthly memberships renew automatically according to the Membership Agreement unless cancellation is submitted within the required cancellation period.

Annual memberships are prepaid and currently receive a discount from the aggregate regular monthly membership price. When an annual member qualifies for an early-cancellation refund, any refund will be calculated using the regular monthly membership rate for months used, rather than the discounted monthly equivalent of the annual membership.

For example, where the annual membership is $1,530 and the regular monthly membership is $150, cancellation after four months would result in $600 being attributed to the four months used:

4 × $150 = $600
$1,530 − $600 = $930 potential refund

The $250 enrollment fee remains non-refundable and is not included in this calculation. Eligibility for an annual refund remains subject to the Membership Agreement, applicable law, outstanding balances, and other applicable terms.

Payment Policies & Methods

OPPMed is a direct-pay medical practice for applicable individual services. Payment is generally required at the time of registration, enrollment, ordering, or appointment booking unless another arrangement has been approved. OPPMed does not guarantee insurance reimbursement. Patients who intend to seek reimbursement through an HSA, FSA, insurance plan, employer benefit, or other program are responsible for determining whether a service qualifies.

Acceptance of payment does not obligate an OPPMed clinician to provide medically inappropriate care or issue a requested prescription, order, certification, or referral. Prices may change prospectively. Changes do not retroactively alter completed transactions unless otherwise stated.

Late Fees & Missed Appointments

Patients are responsible for attending appointments at their scheduled time. A missed appointment or failure to connect within the applicable appointment period may be treated as a completed reservation of clinician time and may remain non-refundable. Repeated missed appointments, failed payments, chargebacks, or abuse of the scheduling system may result in restrictions on future scheduling. OPPMed may waive or modify administrative charges at its discretion where circumstances warrant.

03 | Memberships

Direct Primary Care Membership Agreement Notice

Enrollment in OPPMed Direct Primary Care is governed by the Direct Primary Care Membership Agreement accepted during enrollment. The Membership Agreement establishes the services included, membership fees, enrollment fee, renewal terms, cancellation procedures, exclusions, patient responsibilities, and other conditions governing membership. A copy of the applicable agreement is provided electronically to the email address supplied during registration. Members should retain the agreement for their records. If you cannot locate your Membership Agreement, contact OPPMed to request another copy.

Automatic Renewal Notice

OPPMed monthly memberships automatically renew each billing cycle until canceled in accordance with the Membership Agreement. Monthly billing runs according to the membership billing cycle established by OPPMed. Members wishing to discontinue renewal must submit cancellation within the period specified in their Membership Agreement, currently at least seven days before the end of the applicable monthly billing period, unless applicable law requires a different process.

Annual memberships renew according to the annual renewal terms presented at enrollment and contained in the Membership Agreement. OPPMed will administer automatic renewals in accordance with applicable law.

Membership Cancellation Terms

Membership cancellation must be submitted through an approved OPPMed cancellation method. Cancellation does not retroactively eliminate charges already incurred. Monthly subscription payments and enrollment fees are non-refundable except where required by law. Eligible annual-membership refunds are calculated according to the Annual Membership Refund Policy and the executed Membership Agreement. Termination of membership does not eliminate unpaid balances or obligations arising before termination.

Membership vs. Insurance Disclosure

OPPMed Direct Primary Care membership is not health insurance. Membership provides access to defined OPPMed services described in the Membership Agreement. Membership should not be relied upon as a substitute for health insurance covering services outside OPPMed, including hospitalization, emergency medical care, surgery, specialist treatment, ambulance transportation, or other services not included in the membership. Members remain financially responsible for services obtained from outside providers unless another payer or arrangement covers those services.

04 | Virtual Medical Services

Telemedicine Consent & Limitations

Telemedicine allows patients and clinicians to communicate through electronic technologies when they are not physically present in the same location. By electing to receive telemedicine services, the patient acknowledges that virtual care has benefits and limitations. Certain conditions cannot be adequately diagnosed, evaluated, or treated virtually.

An OPPMed clinician may determine at any time that an in-person physical examination, laboratory testing, imaging, specialist evaluation, urgent care evaluation, emergency department evaluation, or other service is necessary. The clinician's determination that virtual care is inappropriate does not guarantee reimbursement of an appointment fee. Telehealth privacy and security protections remain applicable just as they do to in-person healthcare interactions.

Medical Emergency Notice

OPPMed's virtual clinic is not an emergency medical service. Do not use routine OPPMed messaging, email, scheduling systems, or telehealth appointments for a life-threatening or time-sensitive medical emergency. Individuals experiencing symptoms that may constitute a medical emergency should call 911 or seek immediate evaluation at an appropriate emergency department. OPPMed does not guarantee continuous monitoring of patient messages.

Geographic & Licensure Limitations

Medical services are provided only where legally permissible. Patients may be required to disclose and verify their physical location at the time of the encounter. An OPPMed clinician may decline or discontinue an encounter if the clinician is not authorized to provide the requested service in the jurisdiction where the patient is physically located. Availability of OPPMed services may therefore vary by state, territory, province, country, clinician licensure, service type, and applicable law.

Prescription Policy

A medical consultation does not guarantee that medication will be prescribed. All prescribing decisions are made independently by the treating clinician based on the patient's medical history, clinical findings, available records, identity verification, applicable prescribing standards, federal and state law, and the clinician's professional judgment. OPPMed may decline to prescribe or refill medications that are clinically inappropriate, outside the scope of the service, inconsistent with safe prescribing practices, require an in-person evaluation or additional testing, or cannot legally or appropriately be prescribed through a virtual encounter.

Patients are responsible for providing complete and accurate information regarding current medications, allergies, medical conditions, previous adverse reactions, and other information relevant to safe prescribing. When a prescription is issued, patients may generally select the pharmacy where they would like the prescription sent, subject to applicable law, pharmacy requirements, medication availability, and OPPMed's prescribing procedures.

OPPMed may also identify pharmacies, prescription-discount programs, manufacturer programs, or third-party savings applications that patients may wish to consider for convenience or potential cost savings. Unless OPPMed expressly states otherwise, a referral, recommendation, link, or reference to any pharmacy, prescription savings program, discount card, website, mobile application, or other third-party service does not constitute an affiliation, partnership, sponsorship, or endorsement.

OPPMed does not receive commissions, referral fees, advertising payments, rebates, or other compensation for recommending or directing patients to these resources. Any third-party resource is provided solely as a convenience to patients. OPPMed does not control and cannot guarantee a third party's prices, discounts, eligibility requirements, medication availability, dispensing decisions, delivery services, privacy practices, terms of use, or continued availability. Patients are encouraged to compare available options and select the pharmacy or savings resource that best meets their needs. Prescription pricing, inventory, dispensing, and delivery are ultimately subject to the policies and practices of the selected pharmacy or third-party provider.

Laboratory & Imaging Disclaimer

OPPMed may order or recommend laboratory testing or diagnostic imaging when clinically appropriate. Laboratories and imaging facilities are independent providers unless expressly stated otherwise. OPPMed does not guarantee appointment availability, specimen-collection availability, turnaround time, facility pricing, insurance reimbursement, or the performance of an independent facility. Discounted rates made available through particular networks are subject to availability and applicable program terms. A clinician may recommend additional evaluation after reviewing results.

Third-Party Services

OPPMed may coordinate with independent pharmacies, laboratories, imaging centers, specialists, technology companies, interpreters, mobile service providers, and other third parties. Unless expressly identified as part of OPPMed, these organizations are independent entities responsible for their own services. References to external resources, including prescription-discount services, do not necessarily constitute a partnership, sponsorship, endorsement, or guarantee of pricing or availability.

Identity Verification Policy

OPPMed may require identity verification before providing medical services, issuing prescriptions, ordering tests, releasing records, or completing other sensitive transactions. Verification may include government-issued identification, date of birth, contact information, patient photographs, video verification, account authentication, or other reasonable methods. Failure or refusal to complete required identity verification may result in services being delayed or declined. Fraudulent identity information, impersonation, falsification of medical information, or attempts to obtain services through deception may result in immediate termination of services and may be reported where legally appropriate.

05 | Patient Conduct & Clinical Relationship

Respectful Care Environment

OPPMed is committed to maintaining a professional, respectful, and safe clinical environment. A virtual medical practice is still a medical practice. Patients and visitors interacting with OPPMed clinicians and staff electronically are expected to observe the same standards of behavior that would reasonably be expected inside a traditional physician's office. Professional disagreement is permitted. Abuse is not.

Our Standards of Conduct

OPPMed does not tolerate conduct including:

  • threats, intimidation, or harassment;
  • abusive, degrading, or discriminatory language;
  • profanity directed at clinicians or staff;
  • sexually inappropriate conduct or communications;
  • repeated hostile, aggressive, or demeaning behavior;
  • deliberate disruption of clinical services;
  • fraudulent or intentionally misleading information;
  • impersonation or identity fraud;
  • inappropriate prescription-seeking behavior;
  • unauthorized recording where prohibited by law or OPPMed policy; or
  • threats of violence or conduct creating a reasonable safety concern.

These standards apply to telephone calls, video visits, emails, text messages, portal communications, social media interactions directed toward OPPMed personnel, and other interactions with the practice.

Clinician Right to Decline or Discontinue Services

OPPMed clinicians exercise independent professional medical judgment. No patient, employer, organization, membership, or payment arrangement can require a clinician to provide a treatment, prescription, laboratory order, imaging order, referral, certification, medical determination, or other service the clinician believes is medically inappropriate, unsafe, unethical, outside the clinician's scope, or prohibited by law. Clinicians may decline or discontinue a particular service when appropriate. Payment does not purchase a predetermined medical outcome.

Termination of Services

OPPMed may suspend or terminate a patient's access to non-emergency services for serious or repeated violations of practice policies, including abusive conduct, threats, harassment, fraud, persistent nonpayment, inappropriate use of services, or breakdown of the therapeutic relationship. Serious conduct may result in permanent removal from OPPMed's patient roster.

When an established clinician-patient relationship exists, termination will be handled consistent with applicable professional and legal obligations, including reasonable continuity-of-care requirements where applicable. Termination does not prevent a patient from obtaining healthcare elsewhere and does not affect the patient's rights to obtain medical records as provided by law.

No Tolerance for Harassment, Disrespect or Abuse

OPPMed clinicians and staff have the right to work without harassment, threats, intimidation, discrimination, or abuse. A patient's frustration with a clinical decision, prescription decision, refund determination, scheduling issue, or administrative policy does not justify abusive behavior. OPPMed may immediately end a telephone call or virtual encounter when abusive conduct occurs. Severe behavior, credible threats, repeated harassment, or other serious violations may result in permanent termination from the practice and, where appropriate or legally required, referral to law enforcement or other authorities.

06 | Intellectual Property & Website Terms

OPPMed Intellectual Property

Unless otherwise indicated, OPPMed owns or has authorized rights to the original branding, written materials, educational materials, graphics, designs, workflows, training content, policies, website materials, and other proprietary content displayed through its platforms. This includes intellectual property associated with:

  • OPPMed
  • OPPMedallion
  • OPPMed Academy

and their respective logos, design elements, programs, educational materials, training products, service materials, and proprietary content. No content may be copied, reproduced, republished, distributed, sold, modified, or commercially exploited without prior written authorization except as permitted by law.

Trademarks, Patents & Copyrights

OPPMed names, logos, service names, program names, designs, educational products, and other identifiers may be protected by United States and international trademark, copyright, patent, trade-secret, or other intellectual-property laws as applicable. Use of an OPPMed name or logo without authorization does not create any ownership or license. OPPMed reserves all rights in its intellectual property.

Where a mark is federally registered, OPPMed may identify it using the appropriate registration symbol. Unregistered marks may be identified using applicable trademark designations. Patent-protected or patent-pending technologies, methods, or systems should be identified by their applicable patent or application information where appropriate.

Terms of Use

The OPPMed website is provided for lawful informational, administrative, and service-access purposes. Users may not attempt to gain unauthorized access to systems, interfere with website operations, introduce malicious code, scrape protected content for unauthorized commercial purposes, impersonate another person, misuse patient systems, or use OPPMed intellectual property without authorization. OPPMed may restrict website access when necessary to protect its systems, patients, clinicians, employees, intellectual property, or legal rights.

Disclaimer of Medical Advice

General information published on the OPPMed website, blog, educational materials, social media, or public resources is provided for informational purposes. Viewing website content does not establish a physician-patient relationship. Public educational information is not individualized medical advice and should not be used as a substitute for evaluation by an appropriately qualified healthcare professional.
